Four different operations, often confused

Augmentation adds volume with an implant or your own fat. Reduction removes tissue to relieve physical symptoms and reshape. A lift, or mastopexy, repositions the breast and nipple higher without necessarily changing volume. Reconstruction rebuilds the breast after cancer surgery.

Patients frequently arrive asking for augmentation when what they actually need is a lift, because volume loss after pregnancy and breastfeeding presents as deflation rather than true smallness. Putting an implant into a breast that has descended produces a heavy, low result. Examination at the consultation settles which of the four, or which combination, is right.

Implants versus fat transfer

Implants give a predictable, controllable increase, typically one to two cup sizes or more, with a reliable shape. Modern cohesive silicone gel implants from established manufacturers come with a documented warranty, and the specific make, model, size and serial number are recorded in your file and given to you.

Fat transfer uses liposuction from the abdomen or thighs, processed and grafted into the breast. It gives a softer, subtler increase of roughly half to one cup size, with no implant and the bonus of contouring the donor area. Around thirty to forty percent of grafted fat does not survive, so a second session is sometimes needed. It suits patients wanting a modest, natural change who have enough donor fat.

Choosing size and implant placement

Size is decided by measurement, not by a cup letter. Chest base width, existing tissue thickness and skin quality set the range of implants that will look proportionate and be supported long-term. Within that range you try sizers in a bra and choose.

Placement is under the muscle, over the muscle, or dual-plane. Thin patients generally do better with sub-muscular or dual-plane placement, which gives more soft-tissue cover over the upper edge of the implant and a more natural upper slope.

Breast reduction: a symptom operation as much as a cosmetic one

Breast reduction has one of the highest satisfaction rates of any operation in plastic surgery, because it resolves genuine daily symptoms: neck and shoulder pain, grooving from bra straps, recurrent rashes in the fold, difficulty exercising and difficulty finding clothes that fit.

The operation removes glandular tissue and skin, and repositions the nipple-areolar complex on its blood supply. Scars follow a vertical or anchor pattern, and reduction patients typically accept them readily because the symptom relief is immediate. Where symptoms are well documented, part of the cost may be claimable from insurance.

Recovery

1. Days 1–3: one night in hospital, supportive surgical bra, arms kept low.
2. Days 7–10: most patients return to desk work. Driving comfortable at around ten days.
3. Weeks 2–6: surgical bra continues day and night. No overhead or chest exercise.
4. Weeks 6–8: normal bras resume; implants settle into position, upper fullness softens.
5. Months 3–12: scars mature and fade. Final shape assessed at six to twelve months.

Safety, screening and long-term follow-up

Implants are not lifetime devices. They do not need routine replacement on a schedule, but they should be monitored, and revision at some point in a patient’s life is a realistic possibility that is discussed before surgery rather than after.

Baseline imaging is arranged where age or family history indicates, mammography remains possible with implants using additional views, and any new lump or change is investigated rather than attributed to the surgery. Follow-up in this clinic continues at twelve months and annually thereafter for implant patients.
